Update: 6/22/18
Since I was quick to update when things went wrong, I must update what went right, thanks to make ager "Jay".
Early Monday morning (6/11/18), I received a voicemail from Jay addressing my complaint from over the weekend. He explain where the mistake was made (on my part and EOS's part) and he also let me know steps he took fix the matter, as well as things I could do to ensure I did my part in the future.
I was busy and did not call back, but the next day he called again trying to reach me. I did not make it up there until Thursday, and he was there to help me remain calm as he explained what was done and what would be done.
In that I was satisfied, an because of that I am putting back on two stars for effort on behalf of EOS on their manager Jay for customer service and the ability to maintain customer relationship.

Go to LA Fitness. Do not ever give them your checking account or debit card info. Pay cash. I have been having payment issue ever since I went to EOS.
On Thursday I finally was told they have monthly payments, and when asked before, I was told they did not. I asked for them to put me on the monthly plan, waive the fee, which he said was already waived. I paid for my account and brought it up to date. I also asked if I could pay for the month ahead and was told I had to wait until the end of the month since an additional fee for annual membership would be taken out. Then yesterday, they randomly took out $10.00. I called a little after 8 and someone lady answered. I asked her why was the $10 taken out. She never checked asked for my info and said " I can not waive late fees, you have to call back on Monday sine that was when managers are here. I said to her "you were not listening. Listening is a skill. I simply want to know why was $10 taken out of my account." Before I could finish my sentence she cuts me off and finally asks for my number. I told her never mind I'll go there on person. Her rudeness was despicable!
I checked my other checking account this morning, as I do daily, and EOS has taken out an additional $37 out of my other checking account! I called right away and spoke to a much sweeter young lady (as angry as I was, she remained calm and professional whoever she was and she answered all my questions). She said here was a reversal fee, plus late charge but she does not see $10, while I am seeing it.
At this point, I never want anything to do with this place. I should not have to wait until Monday to see a manager and they are ridiculously using a third party that makes great effort to rack up charges on a customer. They count on it that is why hey push bi-weekly and some have lied that here is no monthly.
I had LA fitness for 3 years in Illinois, and never had a single problem, and they have a Grace period and I do think they charge a late fee. Never had one!
At LA fitness, you pay for that in advance so you cancel when you want with no extra fees.
The only reason I chose this place, is because it is close to work and they had a deal...with my job. To cancel my membership, I have to give a 30 day notice to EOS or I risk extra fees. Im absolutely disgusted with this EOS!
The staff is still awesome, place still clean, and they did some construction work the last time I visited. It's still convenient as far as location, but I'm going to LA Fitness moving forward once I give them my 30 day notice!

Affordable, convenient, and a little small location. But it has the basic plus some machines inside.
I find it to be rather clean and not so busy between 1&3pm. Lunch time seems busiest.
Customer service is great and the staff are friendly.
One star removed for late charge.
Also, a little annoyed with a particular gym trainer. He put pressure for me to accept service I am not interested in.
